Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety inspection was initiated on 12/10/2021 and concluded on 12/16/2021. There were 42 children present with 5 staff. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the vendor failed to ensure that the tuberculosis screening for staff shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
The two TB screenings on file for staff 1 were not completed within the 30 day window required prior to employment.
a. Staff 1 has a hire date of 06/03/2021. TB screening #1 was completed on 01/29/2021 and TB screening #2 was completed on 09/03/2021.
Plan of Correction: All staff will obtain a TB screening within 30 days prior to employment.
