Inspector Notes

An unannounced renewal inspection was conducted on 12/19/2023 from 4:10 pm - 6:21 pm. At the time of entrance there were 64 school age children in care with 6 staff.
Children were observed during arrival from public school, eating afternoon snack and working on homework and arts & crafts. Dismissal/parent pick-up was additionally reviewed.

Records were reviewed for 5 children and 7 staff.

Violations were found in the areas of administration and staff qualifications. These violations are listed on the violation notice issued to the facility and were reviewed with administrative staff at the conclusion of the inspection.

Standard 8VAC20-780-130-F
Based on record review and interview, the center failed to obtain documentation of additional immunizations once between each child's fourth and sixth birthdays.

Evidence:
1. The immunization record for child 1 was not current as the documentation on file lacked documentation of required immunizations administered once between age four and six years.
2. Administrative staff confirmed that the complete immunization record for child 1 was not on file at the facility.
Plan of Correction: Administrative staff stated that the parent of child 1 will be asked to provide the complete immunization record.
Standard 8VAC20-780-160-A
Based on record review and interview, the center failed to ensure that documentation of the required tuberculosis screening shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The TB screening for staff 1 was completed 06/06/2023 which is after her hire date of 04/04/2023.
2. Administrative staff confirmed that the TB screening had been obtained after her hire date.
Plan of Correction: Administrative staff stated that all staff will obtain a TB screening that has been completed within 30 days prior to hire.
Standard 8VAC20-780-340-D
Based on observation, staff interview and record review, the center failed to ensure that the qualified program leader shall supervise no more than two aides.

Evidence:
1. A total of 4 staff were observed supervising a group of 50 school age children. Only one of these 4 staff ( staff 2) meets program leader qualifications according to written documentation reviewed in staff records.
a. The inspector observed this group of children supervised by staff 1, staff 2, staff 3 and staff 4. These children remained with the same 4 staff throughout the inspection.
2. Administrative staff confirmed that the only program leader qualified staff for this group was staff 2.
Plan of Correction: Administrative staff stated that staff will work on completion of training to meet required program leader qualifications.
