Inspector Notes

An unannounced monitoring inspection was completed on 11/01/2024 as a part of the licensure period. The on-site inspection began at 11:30amand ended at 1:30pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 37 children present and 4 staff. The inspector reviewed 4 children?s records and 4 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The form used to record child #2's immunizations was not signed by the child's physician stating he is "adequately immunized."
Plan of Correction: The director will contact the child's parents and request an updated immunization record with the physician's signature stating the child is adequately immunized.
