Inspector Notes

An unannounced monitoring inspection was completed on 04/04/2025 as a part of the licensure period. The on-site inspection began at 11:00amand ended at 1:00pm.The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 46 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
Tuberculosis (TB) tests/screenings are required at least every two years from the date of the first initial screening or testing. Staff A's last documented TB screening was dated 09/06/2022.
Plan of Correction: Staff A obtained a TB test on 04/07/2025 and it is not filed in his record.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
One of the free standing cabinets that contained 2 bottles of cleaners and a bottle of disinfectant spray were not locked.
Plan of Correction: The program director will remind staff the all chemicals should be locked when not in use. The director will monitor for compliance.
