Inspector Notes

An unannounced inspection was conducted on 5/4/2022. Upon arrival to the center one of the four year old classes was having graduation. This class had 15 children and two staff. Each class went outside at their scheduled time. The children have been busy making mother's day gifts and end of the year projects. There are eight classrooms with ages ranging from 21/2 years to five years, with a total of 67 children present with 11 classroom staff and three office/aide staff. A total of five children's records and five staff records were reviewed.

Based on an interview with the program director and a review of documents, the facility failed to ensure that all persons who indicate on a sworn statement having lived outside Virginia in the last five years. and who work alone with children, have completed all required background checks prior to employment.
Evidence:
1. There was no out-of-state sex offender background check conducted for staff #3 prior to the hire date of 1/2022.
Plan of Correction: The director requested and received the out-of-state background check on 5/6/22.
The director will ensure that all staff that have lived outside of the state of Virginia in the past five years has all the required background checks from that state.
