During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/8/2025. The on-site inspection began at 10:15 am and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 65 children present and 15 staff. The inspector reviewed five children?s records and five staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/28/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-770-40-D-1-a
The center is required to have each officer of the corporation and agent obtain a completed national criminal background check, a search of the central registry and a completed sworn statement whenever a new officer is appointed. Before the end of 30 days after the change.
Board officers # 2-4 and agents # 1-2, new officer for ( ) months, did not have documentation of requesting a national criminal background check, a search of the central registry and a completed sworn statement .
Plan of Correction: CRS, background check and sworn statement to be completed .
Standard 8VAC20-780-160-A
Each staff shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming in contact with the children.
Staff #3 had no documentation and had been employed for two months.
Plan of Correction: #3 need to submit doctor note.
Standard 8VAC20-780-240-B
Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff # 3, 4 & 5, who are working with children, has not yet completed the orientation training which is over due by two months.
