Inspector Notes

A monitoring inspection was initiated on 10/7/21 and concluded on 10/12/21. The administrator was contacted by telephone and a virtual inspection was conducted. There were 59 children present, ranging in ages from 3 to 5, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on the review of 5 staff records, the center failed to get out of state background checks for one staff, staff C.
Evidence: One staff member indicates on her sworn disclosure statement that she has lived in 4 other states in the last 5 years. The center does not have any background checks for the staff from other states. The center does have the required Virginia background checks.
Plan of Correction: We did not know we needed to get this. We will make sure we get the required background checks from the other state. Upon further discussion with the staff she has only lived in one state other than Virginia in the last 5 years. We will have her fill out a new sworn disclosure to reflect that.
