During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/06/2025, as a part of the licensure period. The on-site inspection began at 12p.m. and ended at 2:30p.m.. The inspector reviewed compliance in the areas listed above. There were 34 children present and 13 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff #5, has been employed prior to 2021 and currently resides out of state, and does not have documentation of requesting a central registry check from one state.
Plan of Correction: Will request the central registry immediately. Will forward the results to the Licensing Inspector within 10 working days for review. This staff will remain in the presence of staff with current background clearances at all times until the results are received.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff #3 and #4 has been employed for over 2 years. The DOE sponsored orientation course has not been completed.
Plan of Correction: All staff who have not completed the 10 hour preservice will have it completed within one month.
Standard 8VAC20-780-380-A-2
Centers operating five or more hours per day shall have a designated rest period for at least one hour but no more than two hours.

The center's hours of operation is from 8:45a.m. to 2:45a.m. Three children (3 years of age) are present in the "Explore More" afternoon program. A designated rest period for at least one hour has not been provided.
Plan of Correction: Will inform parents and will begin implemented this rest period for any children under the age of 5 years.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

The lists of allergies, sensitivities and dietary restrictions in all classrooms were not kept confidential.
Plan of Correction: We will relocate the lists in all classroom to the inside cabinets.
Standard 8VAC20-780-70
If a reference check is taken over the phone, documentation shall include dates of contact and signature of staff obtaining the reference.

The references in Staff #3's file did not include the dates of contact and signature of staff who obtained them.
Plan of Correction: Will sign and date all references obtained going forward.
