Inspector Notes

An unannounced Renewal Inspection was conducted on October 25, 2023 between the hours of 10:15am and 11:55am. There were classrooms observed with a total of 45 children present ranging in ages from 2 to 5 years with 8 staff supervising. Ratios were in compliance.
The children were observed playing on the playground, singing songs, manipulatives, toileting routines, hand washing procedures, and other organized activities. A complete inspection of the physical plant, child and staff records, fire drill log, and emergency procedures were observed during this inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the provider did not obtain an out-of-state criminal history record check prior to the 1st day of employment.
Evidence:
1. Staff #12 (DOH 8/30/23) does not have documentation of an out-of-state criminal history check on file.
2. Staff #12 moved to Virginia in 2019.
3. The director acknowledged the out-of-state criminal history search had not been completed.
Plan of Correction: I will submit the criminal history search right now.
Violation was corrected during the inspection and search results were received within the hour of inspection.
