Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A mandated monitoring inspection was initiated on August 10, 2021 and concluded on August 12, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 60 children present, ranging in ages from 16 months to 12 years of age, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of staff records, the center failed to ensure that an out-of-state criminal history background check was completed before the first day of employment.

Evidence:
1. Staff #2's sworn disclosure statement, dated 06/14/2021, indicated that they had lived outside of Virginia within the past five years. Date of employment was 06/21/2021.
2. The out-of-state criminal history background check was dated 06/23/2021.
3. Staff verified the date of employment for Staff #2 and the date on the out-of-state criminal history background check.
Plan of Correction: In the future administration will ensure that all required background checks are on file before an employee begins employment.
