Inspector Notes

An unannounced renewal inspection was conducted on 1/18/2023 from 9:00 a.m. to 11:10 a.m. There were 66 children present, ranging in ages from 18 months to 5 years old, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the center failed to obtain documentation of additional immunizations once every six months from children under the age of two year.
Evidence
1. Child #2?s birth date is 5/2/2021.
2. The immunization on file for Child #2 were dated 4/05/2022.
3. Staff #6 confirmed the center had not obtain immunizations for Child #2 every six months for Child #2.

Based on review of documentation, the center failed to ensure that shelter in place procedures were practiced at a minimum of twice a year.
