An unannounced, on-site inspection was initiated on February 27, 2026 completed on February 27, 2026 in response to a complaint received by the licensing office on February 19, 2026 relating to allegations of Administration and Program. This inspection included document review and interviews.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
