Inspector Notes
An unannounced monitoring inspection was conducted from 10:15 am - 1:45 pm. There were 96 children, ranging from three months to five years of age, present with 14 staff supervising. Infant care was observed and older children were observed playing on the playground, listening to a story, preparing for and eating lunch. Lunch served today: chicken burrito, mixed vegetables, pineapple and milk. Reviewed eight child records, six staff records, the written attendance record, written list of allergies, sensitivities and dietary restrictions, eight medications and authorization forms, seven allergy care plans, and the emergency drill log. Standard 22.1-289.035-B-4
Based on review of six staff records, it was determined that Out of State background checks (Child Abuse and Neglect search) were not conducted as required for staff who indicated they have lived outside of Virginia in the past five years. Evidence: the record for staff #2, (Date of hire: 2/8/2022), who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of child abuse and neglect search request.
Plan of Correction: Review and update staff files for Out of State background checks. Management will review monthly and update as needed.
Standard 8VAC20-780-160-A
Based on review of six staff records, it was determined that not all staff had documentation of a negative tuberculosis (TB) test/screening completed within the last 30 calendar days of the date of employment. Evidence: the record for staff #1 (date of hire: 1/2/2024) had documentation of a negative TB test/screening dated: 11/6/2023; the record for staff #5 (date of hire: 8/28/2023) had documentation of a negative TB test/screening dated: 9/7/2023.
Plan of Correction: Review and update staff files of TB test and screening. Management will review monthly and update as needed. New hire may not start until all paperwork is filled out prior to Day 1.
Standard 8VAC20-780-160-C
Based on review of six staff records, it was determined that not all staff had documentation of a negative tuberculosis (TB) test/screening completed at least every two years from the date of the initial test or screening. Evidence: the record for staff #2 had a negative TB test/screening dated: 2/2/2022.

Standard 8VAC20-780-245-L
Based on documentation review, and staff interview, the facility failed to ensure that there is always at least one staff member on duty who has obtained instruction in performing the daily health observation of children. Evidence: there was no documentation of, and staff stated that they did not have, anyone on staff with the daily health observation training.
Plan of Correction: Review with all staff about conducting Daily Health Observations upon all students arriving each day and marking on company Face to Name sheet.
Standard 8VAC20-780-500-A
(Repeat Violation) Based on observation, not all handwashing procedures were followed, as required. Evidence: an infant was observed during a diaper change and their hands were not washed with soap and water afterwards. Staff did not was their hands after changing a baby?s diaper.
Plan of Correction: Review with all staff that infants though school age should be washing their hands after all diaper changes and bathroom usage, before and after meals, upon entering the classroom on arrival and from outside and when messy.
Standard 8VAC20-780-70
Based on review of six staff records, it was determined that not all records contained all required information. Evidence:
1) The record for staff #1 did not contain documentation of two reference checks;
2) The records for staff #2 did not contain documentation to demonstrate that the individual possesses the education or certification required by the (lead teacher) job position, and was observed working alone in the two year old classroom;
3) The records for staff #2 and staff #4 did not contain documentation of orientation training
