Inspector Notes

An unannounced inspection was initiated on 8/20/2024 and completed on 9/5/2024 in response to a complaint received by the licensing office on 8/15/2024 relating to the physical plant. The inspector reviewed compliance in the areas listed above. This inspection included document review, a tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/12/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Based on observation, interviews and a review of documentation, not all areas of the center inside and outside were maintained in a clean, safe and operable condition. Evidence: the HVAC system for part of the building including the infant and toddler classrooms, kitchen and office area has not been operational since approximately June 6, 2024. On or around June 25, 2024 an emergency work order for portable AC units was placed, and they were installed in the infant and toddler classrooms and in the kitchen/front office area. The portable AC units have cords and cables that are accessible to children in care, and the unit in the infant room has a cable cord cover on the floor that is a crawling/tripping hazard. The units extract condensation, that pours into the food prep area sinks in the infant and toddler classroom, so the sinks cannot be used at this time.

Standard 8VAC20-780-280-B
Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: the kitchen closet door was unlocked, due to cables from the portable AC unit, and there were multiple bottles of hazardous substances such as cleaning materials inside. There was a bottle of bleach water sitting out on top of the diaper changing table, unlocked, in the toddler classroom.

Based on observation, the facility failed to ensure that cribs were placed where objects outside the crib were not in reach. Evidence: the portable AC unit in the infant room was plugged into an outlet above 2 cribs and the cord was in reach. The cribs were not labeled ?out of use.?
