During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 9/9/2025. The on-site inspection began at 10:30am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 44 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within one (1) business day from today, which will be the close of business on 9/11/2025. A POC submitted after this date will not appear on the public website.
Violations
3Staff #2's updated TB screening results were overdue by five months.
A poisonous plant was growing along the exterior of the chain link fence bordering the Preschool playground. Several branches containing poisonous berries have extended through the fence into the children's play area near the slides and are accessible to children.
Long Term: Work order has been placed and escalated to remove the entire tree from the property.
On 9/9/2025 on the Preschool playground, a group of two year old children were observed playing near a poisonous plant with berries that had grown through the chain link fence. Staff #4 was supervising the children at that time and was unaware the plant and berries were poisonous. Management was aware of the plant's toxicity and submitted a work order on 9/5/2025 after concerns with children attempting to eat the berries. There were no actions taken to prevent children's access to the poisonous plant and berries or to inform staff of the poisonous plant.
Long Term: Work order has been placed and escalated to remove the entire tree from the property.
