Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and virtual observations. An on-site inspection was not conducted at this time.

An announced renewal inspection was conducted at 11:00am. The Director was contacted by phone to initiate the inspection. There were 43 children and 8 direct-care staff present. Ratios and supervision were in compliance at the time of the inspection. The facility appeared to be clean and well maintained, and classrooms were well supplied with toys and equipment for the children. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Based on records reviewed, it was determined that 1 of 4 staff records did not include required background checks. Evidence: Staff #7 - Did not have a completed out-of-state sex offender registry check and a out-of-state central registry check. Date of hire was 8/15/21.
Plan of Correction: The checks will be completed and results sent to the inspector for verification.
