Inspector Notes

A complaint investigation regarding staff:child interactions was conducted on 6/17/2022. All applicable staff interviews were completed on this day. The Inspector also reviewed all applicable administrative information that was available on site.

After careful review of all available information, this complaint was deemed not valid. Ongoing compliance with the Minimum Standards is the responsibility of the licensee.

Standard 8VAC20-780-50-B
Based on a review of enrollment records, it was determined that one child's record has not been maintained and made accessible for two years after termination of services.
Evidence:
During this investigation, the record for Child A could not be located on site at the center.
Plan of Correction: Moving forward, all files will be kept on site and easily located for at least 2 years after the child's date of withdrawal. This child's file is on site and will be located next week at the latest.
Standard 8VAC20-780-60-A
Based on a review of children's records, it was determined that the center does not have at the center one child's record that contains all required information.
Evidence:
The enrollment information at the center for Child A does not include the child's documented date of withdrawal from the center.
Plan of Correction: The child's date of withdrawal was found and reported to the Inspector on Monday. The date of withdrawal is now clearly documented in the file.
