During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was completed on 07/18/2025, as a part of the licensure period. The on-site inspection began at 9:25 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were 22 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/25/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-350-B-1
The staff-to-children ratios for children birth to 16 months is 1:4.

The infant room had 9 children in attendance with 2 staff members for a period of 62 minutes, before separating the children. Two of the 9 children were under the age of 16 months.
Plan of Correction: Staff member was reassigned to the toddler room on the day of inspection, we will revise staff schedules.
Standard 8VAC20-780-40-M
The center shall maintain written list of all children's allergies. The list shall be dated and kept confidential in each room where children are present.

The allergy list in the infant room was posted on the outside of the cabinet with no cover, and not kept confidential.
Plan of Correction: All staff were retrained on maintaining confidentiality of child records/medical info. Admin will do a weekly document review.
