Inspector Notes

A monitoring inspection was initiated on July 10, 2024 at 1:18 pm until 2:30 pm and completed on July 11, 2024. There were 35 children present with 6 staff supervising and administrative staff on site. The inspector completed observations of the children in care working on coloring activity, napping, transition from field trip, completing bathroom breaks, playing games, interacting with their peers and staff, and parent pick up procedures. A total of 7 child records and 4 staff records were reviewed. There were two medications reviewed by inspector.

Based on observation and interview, the center did not ensure a copy of the provisional license was displayed by the provider at each public entrance.

Based on review of staff records, the center did not ensure all applicants for employment, employees, applicants to serve as volunteers undergo a background check every five years thereafter.

Evidence: In Staff# 2's records (date of hire: 02/28/08), the Office of Background Investigation (OBI) fingerprint-based background check was dated March 10, 2018.
