Inspector Notes
An unannounced monitoring inspection was conducted today from 9:00am-10:0pm. At the beginning of the inspection there were 11 children directly supervised by 1 staff. The physical plant, 4 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No medications for children were present on the date of inspection. Children were observed participating in group play. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations
10Evidence:
Children 1, 2, 3, and 4's records did not contain documentation of a physical examination by or under the direction of a physician.
Evidence:
A canister of disinfectant wipes were observed in an unlocked drawer in a bathroom used by children.
Evidence:
1. The staff member supervising children in the classroom left 11 children unsupervised for a period of approximately one minute to answer the door.
2. Children were observed coming upstairs from the downstairs classroom unsupervised.
Evidence:
On the date of inspection a mixed aged classroom with children aged as young as two years old had 11 children supervised by one staff member.
Evidence:
There were no written authorizations for diaper cream present in the center on the date of inspection.
Evidence:
1. Child 1's record did not contain:
documentation of viewing the child's identity and age, first date of attendance.
2. Child 2's record did not contain:
documentation of viewing the child's identity and age, first date of attendance, the phone number for one of the listed parents.
3. Child 3's record did not contain:
documentation of viewing the child's identity and age, first date of attendance, the place of employment for each parent listed, the addresses of two persons to contact in the event that a parent cannot be reached.
4. Child 4's record did not contain:
the child's first date of attendance.
5. Child 5's record did not contain:
the place of employment for one of the listed parents, the child's first date of attendance.
Evidence:
The records for child 1, 2, 3, 4, and 5 did not include authorization for emergency medical care, a statement that the center will notify the parent when a child becomes ill, and a statement that the parent will inform the center of communicable diseases to the center within 24 hours.
Evidence:
On the date of inspection no documentation was presented to confirm that any staff member present has obtained within the last three years instruction in performing the daily health observation of children.
Evidence:
1. Bottles containing antacids and vitamins were observed on an open shelf in a classroom with children present.
2. An unlocked cabinet containing various medications, and medicated topical ointments was observed in the office.
Evidence:
There was no practice evacuation drill documented for May 2022.
