During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on 12/4/24. The on-site inspection began at 11:20am and ended at 1:04pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records while on-site. This inspection included document review, tour of the facility and interviews.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A container of bleach cleaning wipes were found on the bottom shelf of a bookcase in the classroom for 4-5 year old children.
Plan of Correction: The cleaning wipes were put into a locked cabinet during the inspection.
Standard 8VAC20-780-510-I
The center may administer prescription medication to a child provided the center has obtained written authorization from a parent or guardian.

A total of 3 medications were on-site that did not have a current written authorization from a parent/guardian. The authorizations on-file for each medication were expired.
Plan of Correction: The Director said they would be requesting updated medication authorization forms from parents and to be signed by a doctor.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

Staff reported that they have not been writing injury reports.
Plan of Correction: The inspector will send a model injury report form. The Director said they will write and maintain copies of injury reports.

Staff records shall include documentation of education.

5 of 5 staff records did not include a copy of educational diplomas needed to qualify each for the position(s) they are working.
Plan of Correction: The Director said diplomas will be put in staff records.
