During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 5/21/25. The on-site inspection began at 10:48am and ended at 12:09pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 5 direct-care staff. The inspector reviewed 4 children?s records and 5 staff records while on-site. This inspection included document review, tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/29/25. A POC submitted after this date will not appear on the public website.
Violations
5Chipped/peeling paint was observed on multiple walls on both levels of the building.
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
A variety of cleaning supplies were found to be kept in unlocked areas that were accessible to children. Household cleaning wipes, air freshener spray and infectant spray were found in an unlocked bathroom cabinet. Household wipes and glass cleaner spray were found on open shelves in another bathroom. and white board cleaning spray was on a bookshelf in one classroom.
The center may administer prescription medication to a child provided the center has obtained written authorization from a parent or guardian.
1 of 2 medications on-site did not have a written authorization to give the medicine. The document on-file gives the public school staff permission, not the center staff.
Evacuation drills were not documented as completed in October 2024 and April 2025. Staff reported that because they conducted a shelter-in-place/lockdown drill those months they thought that met the requirements.
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
2 of 2 written injury reports did not include required documentation. One report did not document the date and time the parent was notified, and the second report did not document how the parent was notified.
