22.1 Early Childhood Care and Education

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 12/9/25. The on-site inspection began at 2:27pm and ended at 3:53pm. The inspector reviewed compliance in the areas listed above. There were 21 children present and 6 staff. The inspector reviewed 2 children?s records and 3 staff records while on-site. This inspection included document review, tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/18/25. A POC submitted after this date will not appear on the public website.

The child day center shall obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 for any founded complaint of child abuse or neglect within the first 30-days of employment.

There was no completed central registry search on-file for Staff #3 who began employment In September 2025.
Plan of Correction: Followed up to complete the central registry search for staff #3.
Standard 8VAC20-780-350-K
In addition to the staff required by rest period ratios, an additional staff member shall always be available on-site to offer immediate assistance.

On 12/9/25, when the inspector arrived to the school, Staff #4 was observed working alone with 21 children, who were napping. There was no additional staff present in the building at the time.
Plan of Correction: All staffs were informed of the requirement. There will be 2 or more staff on premise at all times. [sic]
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information, to include the name, address and phone numbers of 2 emergency contact persons.

The record for Child #9 did not include 1 of 2 emergency contact persons. The father was listed as an emergency contact person. :
