Inspector Notes
An unannounced, on-site code compliance inspection was conducted on 12/2/24 from 10:30 am to 12:15pm. The inspector reviewed compliance in the areas listed above. There were 66 children present with 13 staff. The inspector reviewed 4 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable code sections, and a violation has been documented on the violation notice issued to the program.

The center must submit the request for a search of the child abuse and neglect registry or equivalent registry from any state in which an employee has resided in the preceding five years, prior to the first day of employment.
1. Staff # 1 has been employed at the center for three months.
2.Staff # 1's record indicated that they have resided in another state within the last five years.
3. A request for the out of state child abuse and neglect search was not submitted prior to the first day of employment and to this date.
