The center did not ensure the ratio for one staff to 10 children of 36 months to five years was maintained.
1. Staff # 1 stated that approximately between 4:00pm and 4:05pm Staff # 2 arrived to the 3's classroom. Staff # 2 took the children and started walking towards the playground with the children. Staff # 1stated that Staff # 2 did not verbally communicate that she was leaving the room.
2. Staff # 3 stated that she was in the office when she saw Staff # 2 in the hallway that leads to the playground. Staff # 3 stopped Staff # 2 and counted 14 children, the youngest being 3 years old, with only Staff # 2.
3. Staff # 2 was not in compliance with the ratio requirement of 1 staff to 10 children (3 years old).

The center did not ensure the appropriate supervision of children in care.
1. On 10/24/25 at approximately 4:00pm, the 3's classroom was preparing to go to the playground. Staff # 1 had 9 children in care. 7 children from the 4's classroom were also being brought to go outside with them. Staff # 2 was on her way to the 3's room, during which time Staff # 3 was present with Staff # 1.
2. Between 4-4:05pm, Staff # 2 arrived to the 3's classroom to help Staff # 1maintain ratio with Staff # 1.
3. Video footage showed that by 4:09pm, Staff # 1 and Staff # 2 were on the playground with 14 children, which did not include Child # 1.
4. Staff # 1 stated that she had scanned the room before leaving, completed a number count of how many children were present but she did not complete the required name to face because Staff # 2 had already taken the other children and walked out the room. When Staff # 2 left, Staff # 1 was still in the room with a child that was not Child # 1.
4. At 4:30pm, a parent had come to pick up her child and found Child # 1 sitting alone in the classroom by the cubby shelf. The parent informed Staff # 3.
5. Staff # 3 went to the classroom immediately and found Child # 1 sitting alone by the cubby shelf. Staff # 3 assisted Child # 1 to use the restroom and then took Child # 1 to the playground at 4:40pm.
6. Child # 1 was left unsupervised in the classroom from 4:08 pm to 4:30 pm, for approximately 22 minutes.
