Inspector Notes

A code compliance inspection was initiated on February 28, 2024 and concluded on February 28, 2024. The inspection occurred from 10:05 AM to 11:15 AM. There were 56 children present, ranging in ages from 2 to 5, with 8 staff supervising. Additional administrative staff were present. The children were observed participating in multiple activities to include, reviewing colors, shapes and letters, preparing for and eating snack and large group actives in the fellowship hall. The sample size was 10 staff records. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication.

Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Based on a review of staff records and staff interviews, the provider did not ensure a daily simple health screening and exclusion of sick children by a trained person.

Evidence: Center Director, Staff #1 confirmed there was not a staff member who had the required training in daily health screening. Staff #1 had a certificate dated 1-27-2020, however it expires after three years

Based on observation and staff interviews, the center did not ensure all licensed child day
programs and religiously exempt child day centers, certified preschools, family day system
homes and certain other programs that serve preschool-age children operating in a building
built before 2015 to be equipped with at least one carbon monoxide detector.
