22.1 Early Childhood Care and Education

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 10/9/25. The on-site inspection began at 1:18pm and ended at 3:33pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 staff. The inspector reviewed 5 children?s records and 4 staff records while on-site. This inspection included document review, tour of the facility and interview. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/17/25. A POC submitted after this date will not appear on the public website.

Orientation training shall be appropriate to the age of the children in care, job responsibilities to include, but not limited to, documentation of training to care for a child with special needs.

There was no documentation of staff being trained in the special medical care of Child #9.

Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. Unsafe conditions shall include, but not be limited to, protruding nails.

A wood board with protruding nails was observed on the ground of the upper level outdoor play area.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

There were 2 wall electrical outlets and 2 surge protector outlets found without protective covers in the lower level classroom.

Repeat violation.
The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the center has obtained written authorization from a parent/guardian.

There was no documentation of the parents written authorization for staff to administer the medication on-site for Child #6.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include, but not limited to, the following information.

One injury report did not include the date, time and how the parent was notified, and any future action to prevent recurrence of the injury.
