Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 12/17/2024 and the inspection concluded on January 3, 2025. The on-site inspection began at 11:00 a.m. and ended at 12:30 p.m. The inspector reviewed compliance in the areas listed above. There were 12 children present, 1 staff, and 2 parent volunteers. The inspector reviewed 5 children?s records and 6 parent volunteer records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
