During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 05/06/2025 and completed on 05/06/2025. The on-site inspection began at 10:10am and ended at 10:55am. The inspector reviewed compliance in the areas listed above. There were seven children present with three staff. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

(Repeat Violation) Each person in a supervisory position is required to be certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.

The staff health report on file for Staff #1 was obtained more than a month late.
Plan of Correction: Already on file. The center will ensure that all staff have a completed staff health report on file at the time of employment.
