Inspector Notes

An unannounced monitoring inspection was conducted on 03/07/2024 between the hours of 10:50am and 1pm. The physical plant, 3 staff records, 1 cooperative parent record, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside. Areas of non-compliance are identified in the Violation Notice.

Standard 22.1-289.035-A
Based on record review, the center did not obtain a fingerprint-based criminal history record check every five years for 1 of 4 staff reviewed.

Evidence:
1. The fingerprint based criminal history record check results in the record for Staff #3 were dated 09/11/2023. The preceding fingerprint check results in the record were dated 05/15/2018, more than 5 years prior to the date of the most recent results.
Plan of Correction: The center will ensure all repeat backgrounds are obtained every 5 years.

Based on record review, 1 of 3 staff did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR) within 30 days of the first day of employment.

Evidence:
1. Staff began employment on 1/5/2024.
2. The record for Staff #1 did not contain documentation of first aid and cardiopulmonary resuscitation (CPR) orientation training.
Plan of Correction: Staff will complete training in CPR and first aid.

Based on record review, 1 of 4 written records of injuries did not contain all required information.
