During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/1/2025. The on-site inspection began at 10:15AM and ended at 11:22AM. The inspector reviewed compliance in the areas listed above. There were 38 children present and 12 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/8/2025. A POC submitted after this date will not appear on the public website.

When food is brought from home, the following shall apply: The food container shall be clearly dated and labeled in a way that identifies the owner.

All of the lunch containers for Classroom #1 were missing the date.
Plan of Correction: Have ordered tags that can be updated daily for children's lunch/snack.
Standard 8VAC20-780-70
For staff hired after March 1, 1996, documentation that two or more
references as to character and reputation as well as competency were checked
before employment or volunteering:

Staff #4 who has been employed for one year, did not have evidence of completed references in their file.
Plan of Correction: Two references have been added to the employee's file.
