An unannounced inspection was initiated and completed on January 22, 2025 in response to a complaint received by the licensing office on January 14, 2025 relating to allegations of staff records not being complete. The inspector reviewed eight staff records on-site on January 22, 2025. This inspection included document review and interviews. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Staff members must have complete annual "Staff Health Report" on file.
One of eight staff persons did not have a complete annual "Staff Health Report" on file. Staff #7 had a Staff Health Report on file that was dated for over a year ago.
