During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 29, 2025. The on-site inspection began at 12:20 p.m. and ended at 2:00 p.m. The inspector reviewed compliance in the areas listed above. There were 22 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 9, 2025. A POC submitted after this date will not appear on the public website.

All applicants for employment shall submit to fingerprinting prior to employment.

Staff #4 did not submit to fingerprinting prior to employment. Staff #4 has been employed for six weeks.

The child day center must request a copy of the results of a search of the child abuse and neglect registry prior to employment.

The center did not request a copy of the results of a search of the child abuse and neglect registry for Staff #4 prior to employment. Staff #4 has been employed for six weeks.
Plan of Correction: Staff #4 had a copy of the registry results from a previous job, and based on that, the preschool initially believed it was acceptable for her to begin working. However, we later confirmed that a new search is required.

The following staff records shall be kept for each staff person: documentation that two or more references were checked before employment.

At the time of inspection, there was no documentation that two or more references were checked before employment for Staff #3. Staff #3 has been employed for five weeks.
Plan of Correction: Upon hiring, we had a reference log on file, but we asked Staff #3 to provide two formal reference letters for our records. She submitted the letters on the following dates: First letter: October 1, 2025 Second letter: October 6, 2025
