The center is required to request background checks from any state in which the individual has resided in the preceding five years prior to employment. Staff #1, did not have documentation requesting a central registry check and a sex offender registry check from a different state prior to employment and it is overdue by over 11 months.
Plan of Correction: Program Supervisor made immediate correction to missing documentation.
Both checks were requested on 10/28/2025 and results received by 10/30/2025.
The center will also conduct a full file review to ensure all required background checks are completed in compliance with regulation 8VAC20-770.
