Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection initiated on March 31, 2021 and concluded on April 1, 2021. The provider was contacted by telephone to initiate the inspection. There were 33 children and five staff. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on a review of center documentation, in one of two injury reports reviewed, the center did not ensure that injury reports were documented with all required information.

Evidence: An injury report dated 3/25/2021 did not include documentation of the how the parents were notified of the injury and the time that the parents were notified of the injury.
Plan of Correction: The site staff will ensure notation is made of how and when parents are notified of an injury.
