Inspector Notes

An unannounced, on-site monitoring inspection was completed on 04/16/2025. The on-site inspection began at 11:00 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center shall obtain documentation that each child has received immunizations before the child can attend the center.

Child #2 who has been attending the center for at least a year did not have immunizations on record during the inspection.
Plan of Correction: We emailed the parents during the inspection and received the documents.

Each child shall have a physical examination before the child's attendance or within 30 days after the first day of attendance.
