Inspector Notes
A complaint inspection was initiated on June 26, 2023 in response to a complaint that was received on June 22, 2023 in reference to food allergies and management . Video footage was viewed. Documentation was reviewed and interviews were conducted. The preponderance of evidence gathered during the investigation supports violation of standards regarding the complaint.
Violations were cited.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 07/26/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

1) The record for staff #1, date of employment 11/28/22, did not contain a Sworn Statement.
2) The Assistant Director acknowledged that the record for staff #1 did not contain a Sworn Statement.
Evidence:
When asked to view the allergy list that was posted in the classroom, the date was confirmed as 10/13/22. Child #1, age 3 years, was listed as assigned to a different classroom then the child is currently assigned in.
Evidence:
1) Child #1 is on the center's list of children's allergies, sensitivities, and dietary restrictions as allergic to milk.
2) Review of video footage confirmed that child #1, age 3 years, was served and consumed ice cream on 06/16/2023.
3) The Assistant Director confirmed that Child #1 was served and consumed ice cream on 06/16/2023.
Evidence:
1) The record for child #1, age 3 years, did not contain a written allergy care plan.
2) Interview with the Assistant Director confirmed that the record for child #1 did not contain a written allergy care plan.
