Inspector Notes

An unannounced monitoring inspection was conducted on September 15, 2023 from approximately 10:40 am to 1:40 pm. There were 74 children in attendance and a total of 15 staff present. 8 staff records and 6 children?s records were viewed. All areas of the center were observed including classrooms, playgrounds, and bathrooms. The children were observed playing outside, eating lunch, and taking a nap, while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice sign, and return it to me within 5 business days from today, 09/22/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on record review, the center failed to ensure that each staff shall undergo a criminal background check every five years.
Evidence: The record for staff #6, date of employment 11/28/05, contained a criminal background check dated 06/18/18. A repeat criminal background check was due on or before 06/18/2023.

Based on record review, the center failed to ensure that the results of a central registry finding are received within 30 days of employment.

Based on record review, the center failed to ensure that each staff member shall submit documentation of negative tuberculosis screening at the time of employment and no more than 30 days prior to the time of employment.
