Inspector Notes
An unannounced monitoring inspection was conducted on November 28, 2023 from approximately 11:50 am to 2:30 pm in response to a self-reported incident that was received and occurred on November 15, 2023 in reference to supervision . Documentation was gathered and interviews were conducted in person and via telephone. The preponderance of evidence gathered during the investigation supports violation of standards in regard to the self report.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 12/01/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Violations
5Based on records review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and shall have been completed within the last 30 calendar days of the date of employment.
Evidence:
1. The record for staff #1, date of employment 03/14/2023, contained a tuberculosis screening dated 08/09/2022. The tuberculosis screening was conducted more than 30 days prior to the date of employment.
2. The record for staff #3, date of employment 01/31/2023, contained a tuberculosis screening dated 12/23/22. The tuberculosis screening was conducted more than 30 days prior to the date of employment.
3. The record for staff #5, date of employment 12/12/2022, contained a tuberculosis screening dated 06/17/2022. The tuberculosis screening was conducted more than 30 days prior to the date of employment.
Evidence: The record for staff #2, date of employment 09/14/2021, contained an initial tuberculosis screening dated 04/03/2021 and a follow-up tuberculosis screening dated 10/18/2023. The follow-up screening was due on or before 04/03/2023.
Evidence:
1. Interviews with staff #1, staff #2, staff #3, staff #4, staff #5, staff #6, and staff #7 revealed that on November 15th child #1, age 15 months, was found alone outside of the playground gate.
2. Child # 1 was found standing by the playground gate attempting to push the gate open by staff #5 and staff #6 as they pulled into the parking lot on the school bus.
3. Staff #1, staff #2, and staff #3 were on the playground for approximately 10 minutes with 12 children ages 12 months through 24 months. Staff #1, staff #2, and staff #3 transitioned the class inside the classroom from the playground.
4. Child #1 was alone outside of the playground gate for approximately 3 minutes.
5. The center is located next to a street and shares a parking lot with other businesses.
Evidence:
1. Interviews with staff #1, staff #2, staff #3, staff #4, staff #5, staff #6, and staff #7 revealed that on November 15th child #1, age 15 months, was found alone outside of the playground gate.
2. Child # 1 was found standing by the playground gate attempting to push the gate open by staff #5 and staff #6 as they pulled into the parking lot on the school bus.
3. Staff #1, staff #2, and staff #3 were on the playground for approximately 10 minutes with 12 children ages 12 months through 24 months. Staff #1, staff #2, and staff #3 transitioned the class inside the classroom from the playground.

Evidence:
1. Interviews with staff #1, staff #2, staff #3, staff #4, staff #5, staff #6, staff #7, and administration revealed that on November 15th child #1, age 15 months, was found outside of the playground gate alone.
2. Interviews with staff # 1, staff #2, and staff #3 revealed that the center playground policy was not followed when the class transitioned from the playground into the classroom. Staff #1, staff #2, and staff #3 did not conduct a headcount when they entered the classroom with the children and did not discover that child #1 was missing until staff #5 brought the child to the classroom approximately 3 minutes later.
3. The center's playground policy states "to go inside line the children up at building door, count, make sure # is correct, check off as children walk into the building. Keep the door open until all children have entered the building and all names are checked off...Never close the exit door until you have All the children!"
