Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 6/13/2022. There were 3 children present with 2 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the assistant was missing the VA Preservice Training within 90 days of employment.
Evidence:
No certificate was available to verify that the assistant had completed the training within 90 days.
Plan of Correction: The caregiver will go into the training site and download a new certificate if the original one can not be located.

Based on record review, the provider did not have documentation of obtaining the annual subsidy update course (3 hours)
Evidence:
The provider did not obtain the annual subsidy update training for 2021.
Plan of Correction: The annual update will be obtained within the month.
