Inspector Notes
An unannounced monitoring visit was conducted with the provider. Three children were in care for eight points which is within ratios for supervision. Children were upstairs upon arrival and then we all went into the basement daycare space. The children were playing with housekeeping materials, dinosaurs and pretend food. Reviewed emergency preparation, transportation, records for provider and children in care as well as physical space. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
8Evidence:
The storm door glass was shattered and multiple small shards of glass were hanging on the top of the door frame.
Evidence:
The children were upstairs upon arrival and had access to a kitchen knife on the counter.
Evidence:
A child under two years of age had access to stairs on multiple levels of the townhouse as no gates were in place to prevent the child climbing or falling down the steps.
Evidence:
The provider completes the attendance for all children in advance before they arrive and does not document time in and time out.
Evidence:
Child #1 and Child #2 were missing medical insurance policy number.
Child #2 was missing an updated immunization record (last updated 2/2022)
Child #1 was missing documentation of an annual update
Evidence:
The provider changed a child's diaper after it was soiled and the child did not wash their hands.
Evidence:
The Provider stated that she changes diapers on a blanket.
Evidence:
No drills were conducted the months of February and March 2023.
