During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 2/3/2026 and completed on 2/3/2026, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 11:01am. The inspector reviewed compliance in the areas listed above. There were four children present and two caregivers; the point total was six. The inspector reviewed four children?s records and two caregiver/household members records on-site. This inspection included document review and tour of the facility.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Caregivers who work directly with children must complete the department's annual health and safety update course.

Caregiver #2's annual health and safety update course is overdue by 85 days.

Wood burning fireplaces and associated chimneys must be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.

The annual inspection of the provider's wood burning fireplace is overdue by 7 months.
