Inspector Notes
An annual monitoring inspection was initiated on 02/1/2023 and concluded on 02/1/2023. The inspector was on site at the center from 12:05am to 2:00pm. There were 43 children in care with 9 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and all classrooms were inspected today. Five children?s records and five employee records were reviewed during this inspection. Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.
Violations
6Evidence:
The letter of exemption states that no more than five children under the age of two and one half are allowed at any one time. During the 02/01/2023 inspection, seven infants (six weeks to 16 months) were being cared for by two staff members in the infant Room and eight Toddlers (17 months to 23 moths) were being cared for by two staff members in the toddler Room. The infant and toddler rooms do not have classroom doors that lead directly to the exterior.
Evidence:
During the inspection of five staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Daily health screenings should be completed every three years.

1. The record of staff #1 (employment date: 2/1/2023) did not have documentation of out of state sex offender registry check results and criminal history results.
2. The record of staff #1 indicated that the staff member has lived outside of Virginia within the last 5 years.
Evidence:
The licensing inspector did not see a carbon monoxide detector installed in the center. The center?s administrator stated that a carbon monoxide detector is not currently installed in the building.
