An unannounced monitoring inspection was initiated on 09/24/2025 and completed on 10/24/2025 in response to a self-report received by the licensing office on 09/24/2025 relating to termination policies. The inspector reviewed 1 child?s record and 2 staff records on-site and electronically on 10/01/2025. This inspection included document review, tour of the facility, interviews, and observations.
