An unannounced monitoring inspection was conducted today.
There were 63 school-aged children present with five staff.
The children were playing outdoors and eating a snack.
Five child records were reviewed. Three staff records were reviewed.

Based on a review of the playground the facility failed to ensure that, under climbing equipment or equipment moving parts, there is resilient surface material in accordance with the American Society for Testing and Materials requirements.
Evidence:
1. Under the swings and slide there is 4-5 inches of mulch. Under the monkey bars there is 2-3 of the required 6 inches of resilient surface material.
