An unannounced complaint investigation was conducted on 11/16/2023 from 1:33 pm to 2:55 pm in response to a complaint received on 11/16/2023. The Licensing Inspector reviewed one child record, reviewed attendance sheets, and interviewed staff and family regarding the allegations in the complaint. The evidence gathered during the investigation supports the allegation regarding improperly moving a child from one group to an older child group. The complaint is valid.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews and record review, the center failed to have a written assessment by the program director and program leader prior to assigning a child to a different age group if such age group is more appropriate for the child?s developmental level and the staff-to-children ration and group size shall be for the established age group.
EVIDENCE:
There was no written assessment prior to moving Child 1 into an older age group. Attendance sheets show this occurred on 11/14/2023. The lack of an assessment was confirmed by Staff 1.

Plan of Correction: An assessment sheet has been developed and will be used from now on prior to a child moving into a group at a younger age than usual, as defined by the age groups and ratios in the standards.
