Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/8/2025 and completed on 9/9/2025, as a part of the licensure period. The on-site inspection began at 10:45AM and ended at 1:26PM. The inspector reviewed compliance in the areas listed above. There were 54 children present and 20 staff. The inspector reviewed 5 children?s records on site and 5 staff records on-site on 9/8/2025. Medications were observed during the inspection on 9/8/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #2 updated screening was due 9 months ago. The updated screening has not yet been completed.
Plan of Correction: Provider will schedule a tb test for staff.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. There were 10 children present in the Sunflower classroom, but only 9 were marked present on the attendance record.
