During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 02/04/2025 and completed on 02/11/2025. The on-site inspection began at 2:30pm and ended at 4:00pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and three staff. The inspector reviewed five children?s records and three staff records electronically on 02/11/2025. This inspection included document review, tour of the facility, interviews, and observations

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-G
Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. The medication located at the center for Child #6 was not labeled with the dosage amount and times to be given.
Plan of Correction: We will request a copy of the pharmacy label to add to the child's medication.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #1, who has been enrolled for five months, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: We give the family an allergy action plan to have completed by a doctor and add to the child's record.
