During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 06/27/2025. The on-site inspection began at 12:15pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 239 children present and 21 staff. The inspector reviewed 10 children?s records on-site and 10 staff records electronically on 03/30/2025.

This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #3, who had been attending for six months, did not have documentation of an immunization record on file.
Plan of Correction: We will request the immunization record for the child and add it to the file.
Standard 8VAC20-780-510-E
The center's procedures for administering medication shall include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over-the- counter medication may be allowed with written authorization from the child's physician and parent. The center did not have a medication authorization form allowing the center to administer medication for the prescription medication present at the center for Child #3.
Plan of Correction: We will have the parent complete the required medication authorization form.
