Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. A container of sanitizing wipes labeled with Keep out of the Reach of Children was placed in an unlocked cabinet underneath the hand sink located in the school age classroom. School-aged children were present in the classroom during the inspection.
Plan of Correction: All hazardous items have been relocated to be kept safe and out of reach of children.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Child #4, who has been attending the center for six months and was present at the inspection, requires the use of an emergency medication. Staff could not locate the key to gain access to the prescribed medication during the inspection.
Plan of Correction: Staff have been retrained on having all especially emergency equipment ready and accessible.

Each first aid kit shall be easily accessible to staff but not to children. The first aid kit with the required items for the first aid kit could not be located during the inspection.

REPEAT VIOLATION:
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. The record for Child #5 was missing one of the two required names, address, and phone number of designated people to call in an emergency.
Plan of Correction: Interim Director will connect with parents to complete necessary info.
Standard 8VAC20-780-70
The following staff records shall be kept for each staff person and contain name, address, and telephone number of a person to be notified in an emergency and information about any health problems that may interfere with fulfilling the job responsibilities. This information is required to be kept at the center. Staff #5 was present at the center and supervising children during the inspection. The required information was not present at the center.
